DESCRIPTION:Carolyn Flietstra is the executive and strategic leader of home and community services for Holland Home\, Michigan’s largest not-for-profit senior services provider.  Her areas of oversight include community and inpatient hospice\, certified home health\, private duty home care\, a therapy company\, a palliative care institute\, and other support services. \nCarolyn is the past co-chair of the Michigan Home Health Association Public Policy Committee\, and serves on the National Association for Home Care and Hospice’s Financial Management Association Work Group and the Strategic Health Partners Advisory Board. \nShe has extensive experience leading change which optimizes clinical and financial outcomes.
